Background Obtaining informed consent in the clinical setting is an important yet challenging aspect of providing safe and collaborative care to patients. While the medical profession has defined best practices for obtaining informed consent, it is unclear whether these standards meet the expressed needs of patients, their families as well as healthcare providers. The authors sought to address this gap by comparing the responses of these three groups with a standardised informed consent paradigm. Methods Piloting a web-based ‘reverse’ simulation paradigm, (...) participants viewed a video showing a standardised doctor engaging in an informed consent discussion. The scenario depicted a simulated patient with psychotic symptoms who is prescribed an atypical antipsychotic medication. 107 participants accessed the simulation online and completed a web-based debriefing survey. Results Survey responses from patients, family members and healthcare providers indicated disparities in information retention, perception of the doctor's performance and priorities for required elements of the consent process. Conclusions To enhance existing informed consent best practices, steps should be taken to improve patient retention of critical information. Adverse events should be described in the short-term and long-term along with preventative measures, and alternative psychosocial and pharmacological treatment options should be reviewed. Information about treatment should include when the medication takes therapeutic effect and how to safely maintain the treatment. The reverse simulation design is a model that can discern gaps in clinical practice, which can be used to improve patient care. (shrink)

Happiness can be expressed through smiles. Happiness can also be expressed through physical displays that without context, would appear to be sadness and anger. These seemingly incongruent displays of happiness, termed dimorphous expressions, we propose, represent and communicate expressers’ motivational orientations. When participants reported their own aggressive expressions in positive or negative contexts, their expressions represented positive or negative emotional experiences respectively, imbued with appetitive orientations. In contrast, reported sad expressions, in positive or negative contexts, represented positive and negative emotional (...) experiences respectively, imbued with consummatory orientations. In six additional experiments, participant observers interpreted that aggression displayed in positive contexts signalled happy-appetitive states, and sadness displayed in positive contexts signalled happy-consummatory states. Implications for the production and interpretation of emotion expressions are discussed. (shrink)

To address the multiple grand challenges facing humanity, there is an urgent need for businesses to become more sustainable. This study explores the implementation of sustainability initiatives through an interview-based single case study of an organization in the food and beverage industry. Specifically, this study adopts a Natural-Resource-Based View of the firm to examine the role of organizational culture and structure in the implementation process. It argues that to successfully implement sustainability initiatives, a flexible structure and a green organizational culture (...) grounded in a shared sustainability vision are essential. The findings reveal that, despite the in-practice flat organizational structure of the case organization, there was limited knowledge of the organization's sustainability vision among organizational members across all organizational levels. This could be attributed to a lack of internal communication within the organization, which constitutes a barrier to the successful dissemination of green values throughout the organization's family-like, inclusive, and diverse work environment. This study proposes a three-part approach for practitioners attempting to implement sustainability initiatives: (1) enhance organizational awareness and communication of the sustainability vision; (2) cultivate a green organizational culture; and (3) advocate for a collaborative, inclusive and decentralized approach to fostering sustainability. This approach is vital not only for sustaining the organization's competitive advantage but also for achieving improved environmental and social outcomes. (shrink)

This paper continues the debate about how to distinguish metaphor from metonymy, and whether this can be done. It examines some of the differences that have been alleged to exist, and augments the already existing doubt about them. The main differences addressed are the similarity/contiguity distinction and the issue of whether source-target links are part of the message in metonymy or metaphor. In particular, the paper argues that metaphorical links can always be used metonymically and regarded as contiguities, and conversely (...) that two particular, central types of metonymic contiguity essentially involve similarity. The paper also touches briefly on how metaphor and metonymy interact with domains, frames, etc. and on the role of imaginary identification/categorization of target as/under source items. With the possible exception of this last issue, the paper suggests that no combination of the alleged differences addressed can serve cleanly to categorize source/target associations into metaphorical ones and metonymic ones. It also suggests that it can be more profitable to analyse utterances at the level of the dimensions involved in the differences than at the higher level of metaphor and metonymy as such. (shrink)

Over the past few decades, public anxiety about how people interact with science has spawned cycles of discourse across a wide range of media, public and private initiatives, and substantial research endeavors. National and international STEM (science, technology, engineering, and math) education initiatives and research have addressed how students interact with science and pursue careers in STEM fields. Researchers concerned with adult interaction with science have focused on factors that influence how citizens gather and interpret scientific knowledge and form positions (...) on scientific issues, applications, and/or policy in a politicized democratic milieu. Building from research on how the public interacts with science in and outside of formal education, this study focuses on attitudes toward science among students in 4th, 7th, and 10th grades and their parents. Little research to date has paired the STEM experiences of adults with their children. We find that the extent to which parents are positively oriented toward science significantly shapes their children’s attitudes toward science. Furthermore, between 7th and 10th grades, students with parents holding positive orientations toward science are more likely to sustain positive attitudes toward science. Since the foundation for most adults’ interactions with science develops in the K-12 environment, we demonstrate that the foundation, as expressed in adulthood, may directly affect the ways the next generation of students interacts with science. We offer insights into the importance of developing student learning into the social scientific research on public understanding of science and how important scientific issues of today interplay with society. BackgroundHIV cure research involving cell and gene therapy has intensified in recent years. There is a growing need to identify ethical standards and safeguards to ensure cell and gene therapy (CGT) HIV cure research remains valued and acceptable to as many stakeholders as possible as it advances on a global scale.MethodsTo elicit preliminary ethical and practical considerations to guide CGT HIV cure research, we implemented a qualitative, in-depth interview study with three key stakeholder groups in the United States: (1) biomedical (...) HIV cure researchers, (2) bioethicists, and (3) community stakeholders. Interviews permitted evaluation of informants’ perspectives on how CGT HIV cure research should ethically occur, and were transcribed verbatim. We applied conventional content analysis focused on inductive reasoning to analyze the rich qualitative data and derive key ethical and practical considerations related to CGT towards an HIV cure.ResultsWe interviewed 13 biomedical researchers, 5 community members, and 1 bioethicist. Informants generated considerations related to: perceived benefits of CGT towards an HIV cure, perceived risks, considerations necessary to ensure an acceptable benefit/risk balance, CGT strategies considered unacceptable, additional ethical considerations, and considerations for first-in-human CGT HIV cure trials. Informants also proposed important safeguards to developing CGT approaches towards an HIV cure, such as the importance of mitigating off-target effects, mitigating risks associated with long-term duration of CGT interventions, and mitigating risks of immune overreactions.ConclusionOur study identified preliminary considerations for CGT-based HIV cure across three key stakeholder groups. Respondents identified an ideal cure strategy as one which would durably control HIV infection, protect the individual from re-acquisition, and eliminate transmission to others. Known and unknown risks should be anticipated and perceived as learning opportunities to preserve and honor the altruism of participants. Preclinical studies should support these considerations and be transparently reviewed by regulatory experts and peers prior to first-in-human studies. To protect the public trust in CGT HIV cure research, ethical and practical considerations should be periodically revisited and updated as the science continues to evolve. Additional ethics studies are required to expand stakeholder participation to include traditionally marginalized groups and clinical care providers. (shrink)

In Eco’s work between around 1960 and 1992, “openness” in a modern literary text can mean (a) “permitting more than one interpretation,” and (b) “requiring a good deal of decoding work from the reader,” which is close to my own position. These two aspects of openness are demonstrated using Baudelaire’s Les Chats, in regard to which Eco denies that the text may be cristallin in Lévi-Strauss’s sense, while still requiring constructive effort from the reader. It is apparent that this term (...) is equivalent to Riffaterreʼs textual “monumentality.” Eco does not go into detail about the reader’s work in assembling the text’s global propositional structure. It is left to Riffaterre and myself to detail the various stages of this work, involving comparison of images in order to discover their common underlying generative proposition. In contrast to Riffaterre, I have long suggested that the modern poetic text is built on two such propositions. It is at the stage of relations between text and sociolect that Eco contributes much to modern poetics. Openness (b) seems to be a prerequisite for perceptual change in the reader, produced by contrast between textual structure and its sociolectic context. Riffaterre prefers to remain within the text/intertext/interpretant triad, preventing him from describing the text-sociolect relation, where the propositional innovation of the modernist text takes effect. How, if at all, consciousness can be part of the physical universe remains a baffling problem. This article outlines a new, developing philosophical theory of how it could do so, and offers a preliminary mathematical formulation of a physical grounding for key aspects of the theory. Because the philosophical side has radical elements, so does the physical-theory side. The philosophical side is radical, first, in proposing that the productivity or dynamism in the universe that many believe to be responsible for (...) its systematic regularities is actually itself a physical constituent of the universe, along with more familiar entities. Indeed, it proposes that instances of dynamism can themselves take part in physical interactions with other entities, this interaction then being “meta-dynamism” (a type of meta-causation). Secondly, the theory is radical, and unique, in arguing that consciousness is necessarily partly constituted of meta-dynamic auto-sensitivity, in other words it must react via meta-dynamism to its own dynamism, and also in conjecturing that some specific form of this sensitivity is sufficient for and indeed constitutive of consciousness. The article proposes a way for physical laws to be modified to accommodate meta-dynamism, via the radical step of including elements that explicitly refer to dynamism itself. Additionally, laws become, explicitly, temporally non-local in referring directly to quantity values holding at times prior to a given instant of application of the law. The approach therefore implicitly brings in considerations about what information determines states. Because of the temporal non-locality, and also because of the deep connections between dynamism and time-flow, the approach also implicitly connects to the topic of entropy insofar as this is related to time. Emotions, Embodied Cognition and the Adaptive Unconscious argues for the need to consider many other factors, drawn from disciplines such as socio-biology, evolutionary psychology, the study of the emotions, the adaptive unconscious, the senses and conscious deliberation in analysing the complex topography of social action and the making of things. These factors are taken as ecological conditions that shape the contemporary expression of complex societies, not as constraints on human plasticity Without 'foundations', complex society cannot exist nor less evolve. This (...) is the familiar pairing from complexity theory: path dependency and dynamic emergence. Inter-disciplinary and complexity perspectives need to be incorporated into the social sciences. Routinely, sociologists think of social phenomena as a distinct field, expressed in the term: the 'social construction of' without apparent need to refer to other material, biological, psychological, material or ecological conditions or agents. This book shows how the familiar sociological dynamics of identity, solidarity, differentiation and communication are shaped through the persistent interaction of unconscious and affective processing with conscious deliberation in newly emergent contexts. It is this re-expression, not the surpassing, of human characteristics in contemporary social action that needs to re-inform a complex, ecological approach to the theory and methodologies of the social sciences. The book is intended for a postgraduate/research audience and doctoral students to introduce and synthesise inter-disciplinary contributions to research into complexity theory in the social sciences. The book is thought-provoking and informative, wide in scope while also being technically detailed, and still relevant to modem AI [at least as of 1998, the time of writing this review, but probably also at the time of posting this entry here, 2023] even though it was published in 1989. This relevance lies mainly in the book’s advocacy of distributed computation at multiple levels of description, its combining of neural networks and other techniques, its emphasis on the interplay between action (...) and perception, and its particular approach to natural language processing. The criticisms I have occasionally made above are relatively minor, and one of the major shortcomings of schema theory at least at the time of the book’s publication -- namely the lack of a detailed, preferably neural, mechanism for schema instantiation -- is one of which Arbib is well aware. The book is well produced, for the most part clearly written, and has useful summaries at the beginning of each chapter. Both general readers and AI researchers (mainly but not exclusively those working on robotics, vision and neural networks) could benefit from reading the book. (shrink)

I present considerations surrounding pre-reflective self-consciousness, arising in work I am conducting on a new physicalist, process-based account of [phenomenal] consciousness. The account is called the meta-causal account because it identifies consciousness with a certain type of arrangement of meta-causation. Meta-causation is causation where a cause or effect is itself an instance of causation. The proposed type of arrangement involves a sort of time-spanning, internal reflexivity of the overall meta-causation. I argue that, as a result of the account, any conscious (...) process has PRSC. Hence, PRSC does not need to be taken as a stipulation or argued for on purely phenomenological grounds or as a necessary support for reflective consciousness. I also show how it is natural to the account that PRSC is not an additional, peripheral, sort of consciousness, but is intrinsic to all consciousness, thereby fitting claims about self-intimation and co-constitution by various authors, and also being amenable to an adverbial account. As part of this, consciousness of an external object is just the form that current self-consciousness takes, the meta-causal constitution of it being inextricably modulated by the causal relationship with the object. The article also discusses how MCA helps explore issues of for-me-ness, transparency of perceptual consciousness, and possible immediacy and non-relationality of self-consciousness. (shrink)

Ledin and Machin's critique of the use of some current approaches to multimodality for the purposes of critical discourse studies raises some important methodological concerns that need to be addressed. However, both the particular position they develop as well as some of the key points they raise are themselves problematic. In this response, I argue that Ledin and Machin misconstrue some significant aspects of the body of theory they critique and, as a consequence, offer a potentially misleading view of the (...) relevant state of the art in multimodality. I suggest that pursuing closer alignments with theoretically more nuanced accounts of multimodality would allow the account that Ledin and Machin propose to be revised in ways better suited to the task of conducting empirically-driven multimodal critical discourse studies. GUM is a linguistically-motivated ontology originally developed to support natural language processing systems by offering a level of representation intermediate between linguistic forms and domain knowledge. Whereas modeling decisions for individual domains may need to be responsive to domain-specific criteria, a linguistically-motivated ontology offers a characterization that generalizes across domains because its design criteria are derived independently both of domain and of application. With respect to this mediating role, the use of GUM resembles the adoption of upper ontologies as tools (...) for mediating across domains and for supporting domain modeling. This paper briefly introduces the ontology, setting out its origins, design principles and applications. The example cases for this special issue are then described, illustrating particularly some of the principal differences and similarities of GUM to non-linguistically motivated upper ontologies.
